I declare my will to join the OPZZ Konfederacja Pracy and assume the rights and obligations arising from the Statute and provisions of the Union's authorities.I declare that regardless of my possible (including future and parallel) membership in a trade union other than the OPZZ Confederation of Labor - my will is that for all purposes defined by the labor law, including but not limited to determine the size and representativeness of the labor union in the workplace count me exclusively in the OPZZ Confederation of Labor. This is valid until explicitly revoked.

Information clause: The OPZZ Confederation of Labor basic organization operating in the above-mentioned workplace (or the National Board of the Union in special cases), as the administrator of personal data, collects personal data of its members obtained during the signing of the membership declaration and in the course of trade union membership ("Personal Data"), in particular: name(s) and surname, gender, e-mail address, telephone, place of work. These data may be transferred to all instances of the OPZZ Confederation of Labor for the purpose of conducting the activities of the Union.
